Please follow the instructions below to sign up for our pre-authorized payment method. 

1. Enable cookies.
2. Click continue to accept the terms and conditions. 
3.  Complete the form with the exception of your name. The name will be automatically populated after completion. 
4. After you have completed the form, a verification email will be sent to you for security purposes. Click “Confirm my email address”.
5. A confirmation document will be sent which is password protected. Contact our office to retrieve the password.
